Scarless Eye Bag Removal (Under-Eye Fat Removal) in Seoul
Medically reviewed by Dr. Lee Young-geun, board-certified plastic surgeon · Last reviewed:
The simplest of the under-eye operations — and the easiest to get wrong. Removing too much fat is not reversible, and an eye that has been over-hollowed at thirty looks older at forty than it would have done untouched.
Quick answer
Under-eye fat removal is scarless eye bag surgery: the bulging fat is taken out from inside the lower eyelid, leaving no mark on the skin. It suits younger patients whose skin is still firm and who have no tear-trough groove; with a groove or loose skin, a different operation is the better choice.

Overview
What under-eye fat removal actually solves
Under-eye fat removal (눈밑지방제거) is the right operation for one specific patient: someone with a genuine fat bulge, good skin tone, and little or no groove beneath the bulge. That is most often a patient in their twenties or thirties whose eye bags are hereditary rather than age-related.
The procedure is performed entirely from inside the lower eyelid, so there is no external scar and no change to the position of the lid margin. The three fat compartments are exposed and a measured amount of the herniated portion is removed.
The honest limitation of this operation is that it only subtracts. If a tear trough is present and fat is simply removed, the bulge flattens but the shadow remains — and in some patients the under-eye looks more hollow with time. Where that risk exists, Dr. Lee will recommend fat repositioning instead, even though it is the longer operation.
Terminology
What under-eye fat removal is also called
This is the operation people mean when they ask for eye bag removal with no visible scar. It suits younger under-eyes where the skin is still tight.
- Transconjunctival fat removal
- Scarless eye bag surgery
- Transconjunctival lower blepharoplasty
- Eye bag removal
- Puffy eye correction
- Under-eye bag removal
If you were told one of these names abroad, you are on the right page — they describe the operation Dr. Lee performs as under-eye fat removal (눈밑지방제거) here in Sinsa, Gangnam, Seoul.
Candidacy
Who this procedure is — and is not — for
We would rather turn an operation down than perform one that will not help you. These are the honest boundaries of this procedure.
A good candidate
- Hereditary eye bags appearing in the twenties or thirties
- A clear bulge with firm, elastic lower eyelid skin
- Little or no groove beneath the bulge
- Patients who want minimal downtime and no external scar
Better served by something else
- A visible tear trough — fat repositioning gives a better result
- Loose or wrinkled lower eyelid skin — lower blepharoplasty is more appropriate
- Already hollow mid-face, where removing fat would age the eye
- Patients who want 'as much as possible' taken out
The detail that decides the result
Knowing when not to remove
The most valuable judgement in this procedure is deciding how little to take. Fat removed from a lower eyelid cannot be put back without a second operation and a fat graft.
Dr. Lee removes only the portion of fat that herniates forward past the orbital rim, leaves the rest, and sits the patient upright to judge the contour under normal gravity before finishing. Every year of practice has pushed that judgement in the same direction: take less.
Surgical sequence
How under-eye fat removal is performed at SINSA L
Every step below is carried out by Dr. Lee Young-geun himself, from the first marking to the final suture.
Confirm the diagnosis
Is this really a pure bulge? The presence of a tear trough or a sunken cheek changes the recommendation from removal to repositioning.
Transconjunctival access
A small internal incision preserves the orbicularis muscle, the lash line and the lid position, and leaves no external scar.
Compartment-by-compartment assessment
The medial, central and lateral pads herniate to different degrees, and are treated separately rather than as one mass.
Conservative measured excision
Only fat that protrudes beyond the rim is removed. Meticulous haemostasis follows, since bleeding in this space is the main cause of prolonged swelling.
Upright symmetry check
The patient is sat up and assessed in gravity before closure — a step frequently skipped in high-volume practice.

Recovery
What the healing actually looks like
Timelines vary between patients. These are the ranges we tell our own patients to plan around — not best cases.
Every follow-up appointment is with Dr. Lee. If something worries you between visits, you reach the clinic directly — there is no call centre in between.
| Day 0-1 | Cold compression, head elevated. Mild swelling; pain is unusual. |
|---|---|
| Day 2-4 | Most patients return to work. Subconjunctival redness may appear and resolves alone. |
| Week 1-2 | Swelling largely resolved; the contour begins to look natural. |
| Month 2-3 | Final settled result. |
Risks & honesty
What can go wrong
Swelling, bruising, subconjunctival haemorrhage, temporary dryness, asymmetry between sides during healing, and — most importantly — over-removal producing a hollow under-eye, which is why the excision here is deliberately conservative.
Surgical results differ from person to person according to anatomy, skin quality, age and healing response. Every procedure carries risk, including bruising, swelling, asymmetry and the possibility of revision. The content on this site is general information, not a diagnosis or a guarantee of outcome. A face-to-face consultation with Dr. Lee is required before any operation is planned.
How the four under-eye operations compare
Each operation answers a different kind of under-eye. Dr. Lee chooses after examining the bulge, the groove and the skin.
| Operation | Best for | Approach | What happens to the fat | Operating time |
|---|---|---|---|---|
| Under-eye fat repositioning | A bulge with a tear-trough groove beneath it | Inside the eyelid, no external scar | Moved down into the groove, septum reinforced | About 40-60 minutes |
| Under-eye fat removal | A bulge with firm skin and no groove | Inside the eyelid, no visible scar | Taken out | About 30-40 minutes |
| Lower blepharoplasty | A bulge with loose, creped skin | Fine line just under the lash line | Treated, with excess skin trimmed and the eye corner supported | About 60-90 minutes |
| Micro fat grafting | A hollow with no significant bulge | Needle-sized entry points | The patient's own fat added in tiny droplets | About 40-60 minutes |
FAQ
Under-Eye Fat Removal — frequently asked questions
Should I have fat removed or repositioned?
If you have a bulge and nothing else, removal is enough. If you also have a groove running down from the inner corner, or a shadow that stays when the bulge is pressed flat, repositioning will give you a smoother result and a better one in ten years' time. Dr. Lee makes this call at the consultation after examining you, not from a photograph.
Is there really no scar?
There is no external scar. The incision is on the conjunctival surface inside the lower lid and heals without a visible mark.
Will my eyes look sunken afterwards?
Not if the removal is measured. Sunkenness is a consequence of over-removal — one reason only the fat herniating past the orbital rim is taken and the rest is left in place.
Can the fat come back?
Removed fat does not regrow. However, the septum that allowed the fat to herniate remains, and ageing continues, so some patients notice a mild change many years later.
How soon can I wear make-up?
Usually within a few days, as there is no external wound. Avoid rubbing the lid area for two weeks.
How long do international patients need in Seoul?
Typically 4-5 days, as there are no sutures to remove.
Why is this called scarless eye bag surgery?
Because the fat is reached from inside the lower eyelid, through the conjunctiva, so nothing is cut on the outside of the skin. There is no external scar to heal or to hide. The trade-off is that this route treats the fat only — it does nothing for loose skin, which is why it suits younger patients whose skin still snaps back.
Will removing the fat make my under-eyes look hollow later?
It can, if too much is taken or if a groove was already forming and was ignored. That risk is the whole reason this clinic separates removal from repositioning rather than treating every eye bag the same way. If there is a tear trough under the bulge, removal alone is the wrong operation for you and Dr. Lee will say so.
